Metformin is a biguanide compound which lowers blood glucose concentrations in patients with type 2 diabetes. It activates AMP-activated protein kinase -AMPK) in primary hepatocytes when given at 500 uM, resulting in decreased glucose production. Activation of AMPK also increase the expression of the nuclear receptor SHP in liver cells, which inhibits the expression of the gluconeogenic genes PEPCK and glucose-6-phosphatase. Metformin can have different effects on AMPK activity in other types of cells and can signal through AMPK-independent pathways. Metformin may also be useful in the prevention and treatment of cancer.
